Each of our families is given a free essential safety kit, as well as referrals for Home Delivered Meals and In-Home Pharmacy Delivery, at No Additional Cost.
Programs for home-delivered meals are now available for eligible senior citizens throughout Northen Virginia. Home delivery guarantees that homebound seniors receive nourishing meals and receive care in case of falls or other healthcare emergencies.
Eligibility generally consists of being over 60 years, unable to safely prepare their own meals for medical and/or physical reasons and have no other assistance available.
We will coordinate with the various local program specialists to get your loved one referred and evaluated to receive free meals delivered to their home.
Medication non-adherence is a primary cause of medical treatment failures and hospitalizations in the United States. In an effort to improve patient outcomes and reduce hospital readmissions, we have developed an innovative approach to pharmaceutical care and medication management.
Our clients receive a referral for this innovative and efficient service provided by Revolution Meds. Through this service, medications are reviewed by the physician and the pharmacist bills the insurance company directly.
Prescriptions are then refilled on the same day every cycle and delivered to your loved one, the client. Each delivery includes all of your loved one’s medication, sorted in perforated packets with clearly a marked schedule for taking the medication (in the morning, afternoon, or evening).
